This page contains a listing of forms and deadlines for graduate students, faculty, and staff. Failure to meet submission deadlines could result in delayed completion of a student’s graduate degree. Current students should look to the student checklist on MyMichiganTech for a time sensitive list of items they need to complete, and their personalized Degree Completion Timeline for a list of all items needed to complete their degree.

For more information on the application process, please see our How to Apply page. Prospective students can track the status of their application on MyMichiganTech. The online application is used by both domestic and international students to apply to any of our degree programs, certificate programs, or for non-degree seeking status.
The following are forms intended for International applicants that are in addition to the online application process.

- I-20 Transfer Form (for Transferring International Students Only)
Submitting forms in a timely manner is essential for students to complete their degree requirements. Students can find a checklist of items needed at this time and a comprehensive list of items pending and complete for each degree on MyMichiganTech. See our tutorial for a walkthrough.

Forms for all degree seeking students (Masters/PhD)
- Patent, Research and Proprietary Rights Agreement— Submit on MyMichiganTech
- Advisor and Committee Recommendation Form (not required for coursework students unless they want to change their advisor from the graduate program director)
- Degree Schedules and Transfer Credits— Submit one degree schedule per degree or certificate
- Transfer Credits Form—transfer credits from another university to Michigan Tech

Forms for students completing a dissertation, thesis, report
- Petition to Enter Candidacy (allows registration for research credits at the research mode rate)
- Schedule a final oral examination ("Defense") on MyMichiganTech. Please see our student and advisor tutorial for additional assistance.

- Application for readmission and reinstatement—For students who have been inactive for academic, medical, or other reasons
- Continuous Enrollment Course Form (UN5951 - Google form; requires MTU login)

- GSG Travel Grants Application.
- Graduate Academic Dismissal/Suspension Appeal Form—refer to Good Standing & Grading Policies
- Graduate Study/Research Abroad—administered through International Programs and Services
- Management Plan—submit when committee members are in a close, personal relationship [policy]
- Michigan Tech Student Insurance Office—Graduate School policy on health insurance
- MIGS Transfer Form—Graduate students at Michigan Tech may take courses at another Michigan university with approval of both the home and host university

- Request to add a graduate program - a graduate program director may complete this form to admit a current certificate student into a master's program, or a current master's student into a PhD program.

- GTA Proficiencies - for chairs to submit their GTA proficiencies

- PHF Assistantship submission - for student application and faculty recommendation letters
- Recommendation for Appointment to Graduate Faculty Status (includes former permission to teach form)

- Request to add a graduate student to a graduate program - a graduate program director may complete this form to admit a current certificate student into a master's program, or a current master's student into a PhD program.

- Update Graduate Staff - complete this form to update a graduate program director, assistant, or Graduate Faculty Council representative
All Graduate School forms work best in the software that created them. PDF forms work best in Adobe Acrobat or Adobe Reader; DOCX files work best in Microsoft Word. If your browser doesn't display a form when you click on the link, your browser is using an incompatible plug-in to open the file. IT has created a service desk article illustrating several ways to open PDF forms. If you are experiencing problems try:
- Saving the file to your hard drive and open with the newest version of Acrobat Reader rather than clicking on the link and opening in your browser.
- Right-click or option-click the hyperlink
- Save the form on your hard drive.

Academic Year 2023-24 Graduate School Deadlines
All deadlines are at 4pm on the day indicated in the table below. Refer to the Defenses, Theses and Dissertations FAQ to find answers to commonly asked questions regarding important degree milestones.
August 23, 2023 - Wednesday Before Classes Start
Grace period deadline
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School to the Graduate School to qualify for grace period submission for degree completion in fall.
- Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951. Students will pay the resubmission fee when their document is accepted.
- Students who do not meet this deadline will need to maintain continuous enrollment until they complete their degree.
September 29, 2023 - Friday, Week 5
Submit your Graduation Application if you are completing a master's or PhD this semester.
December 4, 2023 - Monday, Week 14
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School for fall completion of degree
- Students who meet this deadline will be eligible to graduate this semester, pending Graduate School review. Please allow six business days for review of documents. Students will have until the Friday of Finals Week to complete any formatting changes required by the Graduate School.
- Students who do not meet this deadline will not be eligible to graduate this semester and will need to maintain continuous enrollment until they complete their degree.
December 4, 2023 - Monday, Week 14
- Submit all other degree completion paperwork (including applicablecertificate degree schedules) to Graduate School—check MyMichiganTech for items needed.
- Submit petition to enter candidacy to be eligible for candidacy for spring.
January 3, 2024 - Wednesday before classes start
Grace period deadline
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School to the Graduate School to qualify for enrollment in UN5951 and degree completion in spring.
- Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951. Students will pay the resubmission fee when their document is accepted.
- Students who do not meet this deadline will need to maintain continuous enrollment until they complete their degree.
February 9, 2024 - Friday, Week 5
Submit your Graduation Application if you are completing a master's or PhD this semester.

May 1, 2024 - Wednesday before classes start

- Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951. Students will pay the resubmission fee when their document is accepted.
- Students who do not meet this deadline will need to maintain continuous enrollment until they complete their degree.
August 5, 2024 - Monday, Week 14
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School for summer completion of degree
- Students who meet this deadline will be eligible to graduate this semester, pending Graduate School review. Please allow six business days for review of documents. Students will have until the Friday of Week 15 to complete any formatting changes required by the Graduate School.
- Students who do not meet this deadline will not be eligible to graduate this semester and will need to maintain continuous enrollment until they complete their degree.
June 7, 2024 - Friday, Week 5
Submit your Graduation Application if you are completing a master's or PhD this semester.
