Forms and Deadlines

For more information on the application process, please see our How to Apply page. Prospective students can track the status of their application on the Michigan Tech application portal. The online application is used by both domestic and international students to apply to any of our degree programs, certificate programs, or for non-degree seeking status.

The following are forms intended for International applicants that are in addition to the online application process.

Additional Forms for Admission

Submitting forms in a timely manner is essential for students to complete their degree requirements. Students can find a checklist of items needed at this time and a comprehensive list of items pending and complete for each degree on their Degree Progress Checklist. See our tutorial for a walk-through.

Forms for all degree seeking students (Masters/PhD)

• Patent, Research and Proprietary Rights Agreement— Submit on your Degree Progress Checklist
• Advisor Recommendation form—Submit to change or appoint an advisor. Coursework students only need to submit this form if they change advisors from their graduate program director.
• Graduation application [Find link in the Graduate Program block of your audit]

Forms for students completing a dissertation, thesis, report

• Advisor Recommendation form—Submit to change or appoint an advisor. Coursework students only need to submit this form if they change advisors from their graduate program director.
• Committee Recommendation form—Submit to appoint a committee. Note that your advisor is not listed on this form; they are automatically members of your committee. 
• Petition to Enter Candidacy (allows registration for research credits at the research mode rate)
• Notify the Graduate School of the date and time of your final oral examination ("Defense") on your Degree Progress Checklist. Please see our student and advisor tutorial for additional assistance.
• TDR-Review (sample form, do not submit)
• Report on Final Oral Examination
• Submission to Digital Commons
• Submission to ProQuest (dissertations and theses only)
• Survey of Earned Doctorates (PhD only)

Special purpose forms

These forms are for students with special circumstances, and are not required for all students.

• Application for readmission and reinstatement—For students who have been inactive for academic, medical, or other reasons
• Continuous Enrollment Course Form - request to enroll in UN5951, a no-fee course which maintains continuous enrollment

• Management Plan—submit when committee members are in a close, personal relationship [policy]
• Michigan Tech Student Insurance Office—Graduate School policy on health insurance
• MIGS Transfer Form—Graduate students at Michigan Tech may take courses at another Michigan university with approval of both the home and host university
• Off Campus Graduate Degree Program Liability Statement
• Parental Leave Request Form
• Request to add a graduate certificate - for enrolled graduate students who wish to add a certificate
• Request to add a master's degree - for enrolled PhD students who want to add a master's along the way in the same department or college.
• Senior Rule Form
• Transfer Credits Form—transfer credits from another university to Michigan Tech
• Withdrawal Form

These forms are for faculty and staff to submit to the Graduate School. Google forms are restricted to members of the graddir-l, gradasst-l, and deptchair-l list. To request being added to the graduate lists, please submit the Update Graduate Staff form.

• Accelerated Courses - submit this form to indicate which courses double count for an accelerated program (master's or certificate)
• Additional Master's Course - submit to indicate which courses double count for an additional master's degree - NOT for accelerated students
• CSCHF Assistantship submission - for student application and faculty recommendation letters
• Curriculum update submission - for 2026-27, the Graduate School will not require a form submission. Contact gradschool@mtu.edu with any questions.
• The DeVlieg Foundation Fellowship - for programs to submit nominations for the fellowship
• Doctoral Finishing Fellowship submission - for student application and faculty recommendation letters
• Finishing Fellowship Reference - for advisors to complete and submit
• Graduate Faculty Status - for deans to submit their annual reviews
• Graduate School Form Submission - for submitting PDF or word doc forms for graduate students or Recommendation for Appointment to Graduate Faculty Status forms to the Graduate School
• GTA Proficiencies - for chairs to submit their GTA proficiencies
• Nomination Form for Outstanding Teaching Award and Dean's Award for Outstanding Scholarship
• MAGS Excellence in Teaching nomination form
• MAGS/ProQuest Distinguished Master's Thesis Award nomination form
• Parental Leave Funding Request
• Recommendation for Appointment to Graduate Faculty Status 
• Recording Graduate Exam Results - for programs to submit completion of coursework exams (select programs), PhD qualifying exams, or PhD research proposal examinations. Replaces using SZAPROT in banner 8.
• Request Funds for Graduate Applicant Campus Visit
• Request for Graduate Tuition-Only Award/Fellowship
• Request to add a graduate student to a graduate program - a graduate program director may complete this form to admit a current certificate student into a master's program, or a current master's student into a PhD program
• Sponsored Programs - forms from sponsored programs
• Termination of Support Request - Request to stop support funding for student
• Update Graduate Staff - complete this form to update a graduate program director, assistant, or Graduate Faculty Council representative

All Graduate School forms work best in the software that created them.  PDF forms work best in Adobe Acrobat or Adobe Reader; DOCX files work best in Microsoft Word. If your browser doesn't display a form when you click on the link, your browser is using an incompatible plug-in to open the file. IT has created a service desk article illustrating several ways to open PDF forms. If you are experiencing problems try:

1. Saving the file to your hard drive and open with the newest version of Acrobat Reader rather than clicking on the link and opening in your browser.
   1. Right-click or option-click the hyperlink
   2. Save the form on your hard drive.
   3. Open the file directly from Adobe Acrobat or Adobe Reader.

April 29, 2026 - Wednesday Before Classes Start

Grace period deadline

Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School to the Graduate School to qualify for grace period submission for degree completion this semester.

• Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951.  Students will pay the resubmission fee when their document is accepted.
• Students who do not meet this deadline will need to maintain continuous enrollment until they complete their degree.
• Only students in a terminal degree are eligible for the grace period.

June 5, 2026 - Friday, Week 5

Submit your Graduation Application if you are completing a master's or PhD this semester. See your Academic Audit for the link and more information.

August 3, 2026 - Monday, Week 14

Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School for summer completion of degree.

• Students who meet this deadline will be eligible to graduate this semester, pending Graduate School review. Please allow six business days for review of documents. Students will have until the Friday of Finals Week to complete any formatting changes required by the Graduate School.
• Students who do not meet this deadline will not be eligible to graduate this semester and will need to maintain continuous enrollment until they complete their degree.

August 3, 2026 - Monday, Week 14

• Submit all other degree completion paperwork to the Graduate School—check your Academic Audit and Degree Progress Checklist for items needed.
• Submit petition to enter candidacy to be eligible for candidacy for next semester.

August 26, 2026 - Wednesday before classes start

Grace period deadline

Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School to the Graduate School to qualify for enrollment in UN5951 and degree completion this semester.

• Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951.  Students will pay the resubmission fee when their document is accepted.
• Students who do not meet this deadline will need to maintain continuous enrollment until they complete their degree.
• Only students in a terminal degree are eligible for the grace period.

October 2, 2026 - Friday, Week 5

Submit your Graduation Application if you are completing a credential this semester. See your Academic Audit for the link and more information.

December 7, 2026 - Monday, Week 14

Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School for fall completion of degree.

• Students who meet this deadline will be eligible to graduate this semester, pending Graduate School review. Please allow six business days for review of documents. Students will have until the Friday of Finals Week to complete any formatting changes required by the Graduate School.
• Students who do not meet this deadline will not be eligible to graduate this semester and will need to maintain continuous enrollment until they complete their degree.

December 7, 2026 - Monday, Week 14

• Submit all other degree completion paperwork to the Graduate School—check your Academic Audit and Degree Progress Checklist for items needed.
• Submit petition to enter candidacy to be eligible for candidacy for next semester.

January 6, 2027 - Wednesday before classes start

Grace period deadline

Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School to qualify for enrollment in UN5951 and degree completion this semester.

• Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951. Students will pay the resubmission fee when their document is accepted.
• Students who do not meet this deadline will need to maintain continuous enrollment until they complete their degree.
• Only students in a terminal degree are eligible for the grace period.

February 12, 2027 - Friday, Week 5

Submit your Graduation Application if you are completing a master's or PhD this semester OR are completing in a future semester and want to participate in spring commencement. See your Academic Audit for the link and more information.

 

April 19, 2027 - Monday, Week 14

Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School for spring completion of degree.

• Students who meet this deadline will be eligible to graduate this semester, pending Graduate School review. Please allow six business days for review of documents. Students will have until the Friday of Week 15 to complete any formatting changes required by the Graduate School.
• Students who do not meet this deadline will not be eligible to graduate this semester and will need to maintain continuous enrollment until they complete their degree. 

April 19, 2027 - Monday, Week 14

• Submit all other degree completion paperwork to the Graduate School—check your Academic Audit and Degree Progress Checklist for items needed.
• Submit petition to enter candidacy to be eligible for candidacy for next semester.
