- Accelerated Courses - submit this form to indicate which courses double count for an accelerated program (master's or certificate)
- Additional Master's Course - submit to indicate which courses double count for an additional master's degree - NOT for accelerated students
- CSCHF Assistantship submission - for student application and faculty recommendation letters

- Recording Graduate Exam Results - for programs to submit completion of coursework exams (select programs), PhD qualifying exams, or PhD research proposal examinations. Replaces using SZAPROT in banner 8.
