First-year Student Permission to Add/Drop Courses

The First-year Student Permission to Add/Drop Courses Formis for first year students to add or drop a course and is orange in color. The form needs to be signed by the instructor and by the students advisor. Once the signatures have been obtained, the for goes to the Registrar's office. A student can either bring the form into the Registrar's Office, room 130 of the Admin Building or it can be scanned and emailed to schedule@mtu.edu. 

Overload Permission

The Overload Permission Form is for students wishing to add on credits above their maximum credits allowed. The student will have to give a reason for their request and have their Academic Advisor as well as the Dean of Students Office sign this form

Release of Information Consent Form

Family Educational Rights and Privacy Act (FERPA) form for the Educational Release of Records for a student. This form is filled out and signed by the student. Once that is done, the form is then brought into the Dean of Students office, room 130 in the Administration Building or scanned and emailed to deanofstudents@mtu.edu

Third and Final Attempt Form

The Third Attempt Form is for students who have failed a course twice or wishes to improve a very low grade. This form is filled out and signed by the student. Once this is done, their Academic Advisor will need to sign the form as well as the Dean of Students office.

Student Withdrawal

The Student Withdrawal Form is for a student who wishes to withdraw entirely from Michigan Tech. Students fill this form out and submit it. Once it is submitted, they will be contacted by someone from the Dean of Students office.
