Spring Co-ops must start between Jan. 2-17, 2023 and end Apr. 21-May 5, 2023
In order to be registered in a co-op course, you must complete all of the appropriate co-op forms. Forms must be downloaded and electronically completed. These forms accept digital signatures and all signatures are required. Career Services will sign their sections upon receipt of the completed forms to firstname.lastname@example.org.
International Students: Once the registration process is complete, you will receive a confirmation email with a link to a Google form for CPT and a new I-20. International Programs & Services will process your I-20 and will notify you when it is ready.

Please send the following forms to your advisor and financial aid to fill out. For graduate students, you may have your academic advisor or your graduate advisor fill out the academic advisor form.
Please sign this agreement form with your employer and yourself. When you submit this form to us, we fill out the MTU signature portion. If the signature is needed by your employer, please note that when you submit the form.
