In these circumstances, we offer Case Management services to help navigate seeking additional resources. To set up a Case Management appointment, please reach out to your established counselor or counseling@mtu.edu to set up an appointment.

Release of Information
If you need confidential records from our center released to an external provider, you will need to complete a release of information form and provide us a written request (usually either by email or fax from the receiving provider). Please contact us if you need any clarification.
