Application for Childcare/Preschool
Fill out one form for each child you would like added to the waitlist. Payment of a non-refundable application fee ($50 for Michigan Tech students and $100 for all others) is required with each application. This payment will place your child's name on the waitlist, but does not guarantee enrollment. Payments can be made by check or money order made out to Gretchen's House Child Development Centers. Return the form in person, by mail or by fax.
This form covers permissions for directory publication, first aid, photo and video, licensing, field trips and health record. A completed form is required for each child enrolled at Little Huskies.
This form is from the State of Michigan Department of Human services and is required for each child enrolled at Little Huskies, to be reviewed and updated on a regular basis. Please fill out all sections of the form.
Each enrolled child must have a health appraisal form on file so we can work together to meet the physical, intellectual, and emotional needs of your child. Some sections are required to be completed by a doctor, nurse, and dentist.
Child and Adult Care Food Program
Little Huskies offers healthy meals to all enrolled children as part of our participation in the U.S. Department of Agriculture's (USDA) Child and Adult Care Food Program (CACFP). The CACFP provides cash reimbursement to our center for serving nutritious meals and snacks. We are required to provide verification of enrollment, attendance and meals/snacks typically consumed by our children and household income information for free or reduced meal/snack reimbursements. Great Start Readiness Program
The Great Start School Readiness Program (GSRP) offers preschool four days per week (Monday through Thursday) from September 1 through April 30 each school year.
