Procedures for Registration Forms

Registration forms help students gain necessary approval from their academic advisor or instructor, and register that approval with the Registrar's Office. Below are the procedures for each type of form, including a link to download a PDF of the form.

Grading Option/Credit Change Form

  1. Decide what grading option you would like:
    • Audit
    • Pass-Fail
  2. Download the Form
  3. Obtain Signatures:
    • Academic Advisor
    • Course Instructor
  4. Return the form to the Registrar's Office by the end of the add period.

Overload Permission Form

  1. Decide how many credits you want to carry
  2. Download the Form
  3. Obtain Appropriate Signature:
    • Dean of Students (If you are on academic probation)
    • Academic Advisor (If carrying more than 18 credits)
  4. Return the form to the Registrar's Office.

Special Approval/Restriction Waiver

  1. Find the restriction you would like to waive
  2. Download the Form
  3. Obtain Appropriate Signatures:
    • Departmental Approval (From department offering course)
    • Academic Advisor
    • If there is an A# code in the listing for this course, you need to get special approval. A1 = Instructor, A2 = Dept, A5 = Career Center, A6 = Graduate School
  4. Return the form to the Registrar's Office before registering for the class.

Time/Exam Conflict Permission Form

  1. Find which two classes conflict
  2. Download the Form
  3. Obtain Appropriate Signatures:
    • Instructor of Course #1
    • Instructor of Course #2
  4. Return the form to the Registrar's Office before registering for the courses.
