Housing Forms

HuskyCard Info/Dining Dollars Deposit Add dining dollars to your Husky Card
Fall 2023 RETURNING Student Late Arrival Form Returning Students can move in August 20th through the 27th. This form is for New Students who need to move in after August 27th. 
Fall 2023 NEW Student Late Arrival Form

New Students will receive assigned move-in times via their MTU email for either August 18th or 19th. This form is for New Students who need to move in after August 19th. 

Thanksgiving Break Early Arrival/Late Departure

Current students may stay in the residence halls during semester breaks (Spring Break and Thanksgiving Break) at no additional fee. Please register in advance if you will stay for the whole break, depart late, or plan to return early.

Winter Break Stay Use if you will be arriving early, or staying late during the Winter Break
Spring Break Early Arrival/Late Departure Use if you will be arriving early, or staying late during the Spring Break
Summer Late Departure Use if you will be staying late at the end of the Spring Semester
Fall Early Arrival/Late Departure Use if you will be arriving early, or staying late during the Fall Break
Meeting Room Reservations Reserve a meeting room in one of the residence halls.
Parking Use to apply for a parking permit
Repair Request Residents may request a repair using this form.
Vending Machine Problems Report Problems with a residence hall vending machine
Dining Hall Promotions Table Reservation To reserve a table for dining hall promotions.
Fall Departure

Residents who will graduate, transfer, withdraw, go on coop, internship, or study abroad need to notify Housing of the departure no later than November 16.

Contract Release Request Students may request to break their housing (room and/or meal plan) contract if a significant change has occurred since the time the contract was signed in a medical, financial, or extenuating circumstance.
How to Dispute a Housing Charge Information about how to file a dispute if you believe you have been improperly charged for cleaning, damages, or any other housing fees.
Meal Plan Change Form  Students may request to change their meal plans.
Alcohol Permit Request Students 21 or older who live in an alcohol permitted house in the Residence Halls can request an alcohol permit.

Fall Departure

Registered Cotenant Tenant Addition or Removal

The contract holder can use this Google form to add or remove a tenant from the apartment. Please submit one form for each addition or removal.

Community Room Reservation Request Form

Daniell Heights contract holders and registered tenants may use this Google form to request to reserve the Daniell Heights Community Room. The Community Room is the side of building 2000 that is also used by the preschool.

Activities Room Reservation Request Form

Daniell Heights contract holders and registered tenants may use this Google form to request to reserve the Daniell Heights Activities Room.  The Activities Room includes couches and the television.  Reservations can be made for any day or time during the week.
