Here are downloadable MTF forms in Portable Document Format (PDF). Simply click on the link, fill in the form (instructions below), then print the form and return it to the Tech Fund at:

General Forms

  • Alumni Update Form
    This form can be used to update your contact information with the Michigan Tech Fund. This can also be done online.
  • McNair Society Declaration of Intent Form
    If you have left or would like to leave a portion of your estate to Michigan Tech, fill out this form and return it to the Michigan Tech Fund's office of Planned Giving. You will then be honored in the McNair Society. Allowing us to add your name to this Society will provide an excellent example for others to follow.
  • Pledge Form
    You may use this form to make a pledge to the Michigan Tech Fund. A pledge is an agreement made by the donor to contribute cash or other assets to a preferred area of support at Michigan Tech.

Form Instructions 

MTU departments Check Request

  1. Click into the "Payee" field. Type your full name; enter; type your address. The field will accommodate several lines. If the request is for a transfer, type "MTU - [Account Number]."
  2. Tab to the next field, "Date." (You can also click into any field.) Enter today's date, either as all numerals (12/1/00) or as text and numerals (December 1, 2000).
  3. Tab to the next field, "Amount." Enter the amount. Do not use a dollar sign.
  4. Tab to the next field, "Purpose." Enter a brief description of the purpose for the check request.
  5. Tab to the next field, "Fund Number." Enter four digits and letters as necessary.
  6. Print.

 

MTF Check Request

  1. Click into the "Payee" field. Type your full name; enter; type your address. The field will accommodate several lines.
  2. Tab to the next field, "Date." (You can also click into any field.) Enter today's date, either as all numerals (12/1/00) or as text and numerals (December 1, 2000).
  3. Tab to the next field, "Amount." Enter the amount. Do not use a dollar sign.
  4. Tab to the next field, "Purpose." Enter a brief description of the purpose for the check request.
  5. Tab to the next field, "Operating Fund 7300." Click to produce a check mark. Tab to the Organization and Account Number fields, entering as appropriate. To reach the "Campaign Fund 7325," tab through the previous fields. The form will allow a check mark in either Operating Fund 7300 OR Campaign Fund 7325, not both.
  6. Print.
