Forms for Faculty Hiring

  • Position Authorization Form
    Before any position can be posted, a position Authorization form must be completed. To complete the form you must seek the appropriate approval through the Dean/Director and then send the completed form to Human Resources.
  • Confidentiality Statement
    Search committee members must sign the Confidentiality Statement.
  • Advertisement and Committee Approval Form II
    When placing an ad for a position, a good faith effort should be made to see that all advertisements reach a diverse population. Faculty positions should be advertised nationally.
  • Purchase Requisition Form
    If you are not using a purchasing card, then a purchase requisition form must be completed to pay for the placement of job advertising.
  • Interview Approval Form III
    This form is used to document the candidates you will be interviewing and the types of interviews you plan to conduct for each candidate. This form is NOT the Applicant Flow Log.
  • Final Selection Form IV
    Provide reasons the non-hire of candidates that were interviewed but not offered the position.
  • Form A Faculty Appointment Recommendation

Special Forms

  • Posting/Search Waiver
    In cases where the suitable candidate is identified and circumstances meet requirements, such as a candidate receiving assistance through the Partner Engagement Program and is qualified for the position, posting the job or searching for additional candidates can be waived.
