Complete this form to start the withdrawal process for the current or upcoming semester. This form only needs to be completed if you intend to drop all of the courses in which you are enrolled. The date you submit this form will impact the refund you may be entitled to. Students may only withdraw during weeks 1-14; withdrawals are not accepted during the final exam period.
Your withdrawal is not finalized until reviewed and processed by the Dean of Students Office, Financial Aid, and Registrar's Office.
