The Assessment Team requests that the following information be sent to Lisa Pignotti whenever an assessment project is submitted to the IRB:
- Name of Survey/Assessment Project
- Name of Person and Department Submitting
- Date submitted
- Effective Dates Approved for Project/Survey
- Status – Exempt from Review or IRB Approved
