Section 1: Employee Information and Verification
This section is to be completed and signed by the EMPLOYEE at the time employment begins. The employee must complete
Section 1 on or before his/her first day of work for pay. Employers CANNOT specify which document(s) they will accept from an
employee.

Section 2: Employer Review and Verification
This section is to be completed and signed by the EMPLOYER (or Authorized Representative) within three (3) business days of the
hire. Examine one (1) document from List A OR examine one (1) document from List B AND one (1) document from List C.

Section 3: Reverification and Rehires
This section is used ONLY when rehiring a former employee. The EMPLOYER (or Authorized Representative) must complete and
sign this section within three (3) business days. Remember, the employer CANNOT specify which document(s) they will accept
from an employee. Human Resources will complete all updates and reverifications in Section 3.
