The purpose of this form is to assist the Institutional Equity Office in determining whether, or to what extent, a reasonable accommodation for an employee with a disability is required to perform one or more essential functions of their job safely and effectively. The employee must initiate this request for an accommodation. The information will be treated confidentially.
If a person feels that his or her treatment has been unfairly discriminatory, he or she is encouraged to file a complaint. These procedures are intended to be used in any case of alleged harassment and/or discriminatory actions on the basis of a person’s race, religion, color, national origin, age, sex, sexual orientation, gender identity, height, weight, genetic information, or marital status, veteran status, disability, etc.
