The purpose of this form is to assist Institutional Equity and Inclusion in determining whether, or to what extent, a reasonable accommodation for an employee with a disability is required to perform one or more essential functions of their job safely and effectively. The employee must initiate this request for an accommodation. The information will be treated confidentially.
If a person feels that his or her treatment has been unfairly discriminatory, he or she is encouraged to file a complaint.
