Reinstatement after Academic Suspension/Dismissal

To request reinstatement after academic suspension or dismissal, please complete the form below. Your request must be submitted at least six weeks prior to the semester in which you wish to be reinstated and will not be considered until all outstanding holds are removed from your record and supporting documentation has been submitted.

 

  • If you have attended any other school since leaving Michigan Tech, please send a copy of your transcript to the Dean of Students Office. If you are currently attending courses, please provide your current grade(s).
  • If you have worked since leaving Michigan Tech, please have your employer/supervisor send us a letter of performance evaluation. This letter can be faxed or mailed.
  • Please contact your academic advisor to formulate a return semester plan. You may email a copy to the Dean of Students Office.
