Undergraduate Student Forms

The following forms are specific to undergraduate co-op students. Review the forms, which appear in chronological order, and take note of their deadlines.

Co-op Checklist

The Undergraduate Co-op Checklist lists things you need to do before registering for your co-op and during your work assignment. Check off the items that you've completed so far, submit a copy to Career Services, and keep a copy for future reference.

Student Information Form

The Undergraduate Student Information Form provides us with your co-op address, contact information, and supervisor's information.

Cooperative Education Agreement

The Undergraduate Cooperative Education Agreement must be completed by Michigan Technological University, the employer, and the co-op student. The agreement outlines the responsibilities of each party; note that the employer section must be completed prior to the co-op start date.

Student Progress Evaluation Form

The Undergraduate Student Progress Evaluation form must be completed by the supervisor and the co-op student at the midpoint of the semester; a form must be submitted for each semester the student is on the co-op assignment.

Co-op Supervisor Form

Co-op students—both undergraduate and graduate—are responsible for ensuring the co-op supervisor completes the supervisor form below.

Final Evaluation Form

The Supervisor Final Evaluation form must be completed by the co-op student's supervisor at the end of the co-op assignment.
